Anonymous Reporting

If you wish to report an HIB Incident anonymously, please complete the state-approved HIB Form 338 (linked above) and leave the section titled "Individual Completing Form" blank. Once the form is complete, you can either mail it to the Board of Education office address below, or put the form in the Drop Box outside the office.
